Andrew P. Daigle, MD is the Associate Medical Director for Community Palliative Consultants in Florida. Jamie Buller, LCSW, ACHP-SW is the Director of Advance Care Planning for Community Hospice of Northeast Florida.

Objectives:

1.       Identify the difference between advance directives and advance care planning.

2.       Describe the benefits of advance care planning.

3.       Discuss how Honoring Choices Florida is different from other approaches.

4.       Identify what physicians can do to ensure patients receive the care they want. 

CME Credit Eligibility:
A minimum passing grade of 70% must be achieved. Only one re-take opportunity will be granted.
